Summary, etc. | Resumé Le concept de psychose hystérique est un ancien concept de la psychiatrie classique. Il est apparu dans les suites de la notion de folie hystérique des aliénistes du XIXe siècle. Après sa description en 1961 par Follin, il a fait l’objet de nombreuses controverses, pour ensuite tomber dans l’oubli. A travers une illustration clinique, nous verrons comment ce diagnostic peut être porté en analysant les différents diagnostics différentiels (psychose aiguë, trouble bipolaire...) et les soubassements psychiatriques et psychanalytiques qui ont présidé à son développement. Les principaux éléments cliniques qui permettent de poser el diagnostic de psychose hystérique sont l’existence d’un délire à thématique sexuelle et persécutive avec dépersonnalisation sans dissociation de la pensée. Le rapport du sujet à son délire qui le fait être plus spectateur qu’acteur, l’existence de traits hystériques marqués, de même que les éléments intrapsychiques d’atteinte de la représentation corporelle sont les principaux éléments discriminants en faveur du diagnostic. |

Summary, etc. | Hysterical psychosis: clinical and historical facetsThe concept of hysterical psychosis is a classical concept in the history of psychiatry. It originated in the notion of hysteric madness introduced by the psychiatrists during the 19th century. After its description in 1961 by Follin, this concept was subject to numerous controversies before being dismissed. A clinical vignette will illustrated how this diagnosis can be carried out by ruling out other diagnoses (esp. psychotic and bipolar disorders) with respect to its psychiatric and psychoanalytical conceptual roots. The main clinical elements which support this diagnosis are: the patient’s attitude towards his/her delusional status making him or her more of a spectator than an actor, the presence of marked hysteric features, as well as body representation disturbances. |
