| Summary, etc. | Resumé Les engagements militaires contemporains ont radicalement évolué depuis deux décennies ce qui donne un aspect spécifique à la pratique actuelle de la psychiatrie en milieu militaire. Les manifestations pathologiques psychiatriques observées sont doubles : au premier plan les pathologies directement articulées au stress aigu et aux états de stress post-traumatiques ; au deuxième plan des troubles des conduites, suicide et addiction surtout. L’observation épidémiologique fine de ces phénomènes leur donne une perspective différente relativement à ce qui était connu jusque là. Par exemple la surconsommation de psychotropes et le phénomène amplifié des conduites suicidaires – 4 fois la moyenne nationale – ont été des surprises pour l’armée américaine. Cela à conduit à des adaptations originales de la psychiatrie militaire en intervention. |

| Summary, etc. | Towards new paradigms in military psychiatry...The contemporary military engagements have changed dramatically over the past two décades which gave specific facets to the current practice of military psychiatry. The encountered psychiatric disorders are twofold: 1) in the foreground, disorders directly linked to acute stress and post-traumatic stress conditions; 2) on a second level, disruptive and conduct disorders, addiction and suicide especially. The epidemiological observation of these phenomena ultimately provided a different perspective as to what was previously known. For example the overuse of psychotropic médication and the increased suicidal behaviors (4 times the national average) came as an astonishing finding for the U.S. military personnel. This led to adaptations of the previous military medical intervention. |
