| Summary, etc. | Le suicide est un événement rare, dont la prédiction reste à ce jour impossible. La littérature a repéré de nombreux facteurs de risque, mais, manquant de spécificités, leur valeur clinique demeure pauvre. Or, s’ils permettent d’évaluer le niveau de risque global d’un patient, ils n’ont pas de valeur prédictive à court-terme, une confusion regrettable existant pour de nombreux professionnels entre évaluation et prédiction. Le risque suicidaire est ainsi surévalué en pratique. C’est aussi la pertinence même des variables associées au spectre suicidaire que nous interrogeons, questionnant ainsi l’application rigide de données globales sur des populations cliniquement distinctes : suicidés, suicidaires et suicidants. Face aux limites du modèle d’évaluation actuel, reposant essentiellement sur des données issues d’études épidémiologiques, nous souhaitons présenter les modélisations théoriques récentes de la crise suicidaire. En effet, celles-ci proposent des données dynamiques, contextuelles, permettant l’articulation d’un niveau de vulnérabilité global et des éléments cliniques permettant une approche phénoménologique du vécu de crise. |

| Summary, etc. | Suicidal behaviors: assessing risk is not predicting acting-outSuicide is a rare event, and his prediction is impossible. Many risk factor are identified, but they tend to suffer of a lack of specificity and their clinical application is limited. Moreover, there is a confusion among many professionals between evaluation and prediction, so the suicide risk is overestimated in practice. We discuss the relevance of the variables associated with suicidal spectrum and the rigidity of application of comprehensive data on clinically distinct populations: completed suicide, person with suicidal ideation only and person who has attempted suicide. To improve our clinical assessment, based primarily on data from epidemiological studies, we present recent theoretical models describing suicidal crisis. Indeed, they provide dynamic and contextual data: overall vulnerability and clinical features can be integrated, allowing a phenomenological approach to the crisis. |
