| Summary, etc. | Les traumatismes psychiques affectent en premier lieu les enfants et les adolescents et ils résultent avant tout de maltraitance physique et sexuelle.Dans le Centre d’Accueil de victimes de violences de Tours, dans lequel j’ai mis en œuvre des recherches et des traitements à partir de 1992, les mineurs représentaient les trois-quarts des patients. Nous avons confirmé la gravité de ce problème lors d’une enquête épidémiologique menée en collaboration avec le groupe INSERM « Santé de l’Adolescent » de Marie Choquet sur plusieurs milliers d’adolescents représentatifs de la population générale : un adolescent sur cinq avait fait l’expérience d’une agression physique ou sexuelle, et si le nombre d’agressions sexuelles n’y excédait pas celui des agressions physiques, par contre leurs conséquences psychologiques excédaient considérablement celles des agressions physiques.Plusieurs syndromes apparaissent après l’expérience de viol ou d’agression. Ils ont une sémiologie distincte et une évolution indépendante. Nous en avons isolé trois : le syndrome traumatique dissociatif et phobique, le syndrome traumatique de reviviscence et le syndrome traumatique de type limite. Ils se déclenchent généralement tous en même temps ou dans une succession rapprochée. Cela confirme ce que les psychiatres français du dix-neuvième siècle avaient relevé à leur façon.Le syndrome de reviviscence est fort mais les deux autres sont souvent plus inquiétants, particulièrement chez les enfants et les adolescents parce qu’ils engendrent des troubles dans le développement psychologique. Le traitement le plus approprié est la psychanalyse existentielle. |

| Summary, etc. | Psychological trauma primarily affects the children and the adolescents and they primarily consist in physical and sexual maltreatment.In the Medico-Judicial Unit Center for Violence Victims in Tours, France, that we joined in 1992 for treatments and research, underage patients represent about three quarters of patients. We confirmed the severity of this problem during an epidemiological survey conducted in collaboration with Marie Choquet’s “Adolescent Health ” group (INSERM) among several thousands of representative adolescents from the general population: one out of five adolescents had experienced physical or sexual assaults, and though sexual assaults did not exceed physical assaults in frequency, on the contrary their psychological consequences did considerably exceed those of physical assaults.Confirming the findings of French psychiatrists during the nineteenth century, several syndromes appear after experiencing rapes or assaults. They have distinct semiology and independent evolutions. We isolated three of them: the dissociative and phobic traumatic syndrome, the re-experiencing traumatic syndrome and the borderline-like traumatic syndrome. They are generally all triggered at the same time or in close succession.The re-experiencing traumatic syndrome is strong but the other two are often more worrying, in particular related to children and adolescents, because they generate disorders in the psychological development. The most appropriate treatment is the existential psychoanalysis. |
