| Summary, etc. | L’émergence internationale du concept de rétablissement dans la schizophrénie vient bouleverser profondément les représentations classiques du trouble. Contre l’idée d’un déterminisme évolutif, le rétablissement désigne la possibilité d’un devenir favorable des personnes qui en sont atteintes. Cet article propose de restituer les grandes avancées qui, sur un plan théorique, clinique et sociétal, ont contribué durant ce dernier siècle au passage du statut de “dément inéluctablement déficitaire”, à celui de “patient-acteur de sa prise en charge”, pour enfin évoluer vers celui “d’usager-citoyen expert de son vécu”. Cet éclairage historique oriente la compréhension des enjeux contemporains du concept de rétablissement dans le champ de la santé mentale en France. Dans cette perspective, le rétablissement est discuté et envisagé dans sa qualité de paradigme, à la lumière notamment de pratiques et modèles de soins internationaux. |

| Summary, etc. | From deterioration to recovery in schizophrenia: history of a change of paradigmaThe international emergence of the recovery concept in schizophrenia deeply changes classical representations of the disease. Against the vision of an ineluctable deterioration, recovery advocates the possibility of a positive become for the majority of persons with the trouble. This article aims to trace the great theoretical, clinical and societal advances that contributed over the past-century to change the status of “a patient with irreversible dementia”, to “a patient-actor of his therapeutic program” to finally evolve into “a user-citizens expert of his lived-experience”. This historical perspective highlights the understanding of contemporary issues on the concept of recovery in the field of mental health in France. Finally, recovery is discussed on his quality of paradigm, particularly in the light of international practices and models of care. |
