| Summary, etc. | La remédiation cognitive assistée par ordinateur (RCAO), dans laquelle le patient est utilisateur direct de la technologie, s’est étendue à diverses pathologies psychiatriques dont la dépression persitante. Son objectif est de restaurer un fonctionnement de qualité par l’intermédiaire de la stimulation interactive des fonctions cognitives altérées. La pratique de la RCAO nécessite que les usagers s’engagent véritablement dans une relation avec l’ordinateur, or ceux-ci sont peu habitués à utiliser les nouvelles technologies au sein de leur « prise en charge ». La médiation de la relation entre le patient et l’ordinateur par un thérapeute s’avère donc nécessaire afin de renforcer la confiance du sujet dans cet outil, de soutenir sa motivation et d’accompagner le développement de la métacognition qui constitue un pivot pour l’impact des résultats sur la vie quotidienne. Bien que la communication non verbale soit modifiée par la présence de l’ordinateur, les aspects empathiques peuvent se développer au cours des rencontres et permettre l’instauration d’une alliance thérapeutique. Nous précisons les modalités d’intervention en RCAO appliquée à la dépression, les enjeux du numérique dans ce contexte et le rôle du thérapeute au sein de ce dispositif de soin. |

| Summary, etc. | Computer assisted cognitive remediation and therapeutic relationship in the treatment of long-term depressionComputer-assisted cognitive remediation (CACR), in which the patient is direct user of the technology, has extended to various psychiatric disorders including long-term depression. Its objective is to restore efficiency of altered cognitive functions through the interactive stimulation. The practice of CACR requires that the patient truly engages in a relationship with the computer. However, users are not accustomed to using new technologies in their medical care. Mediating the relationship between the patient and the computer thanks to a therapist is therefore necessary to improve the confidence of the subject in the tools, support his motivation and develop metacognition which is a pivot for impact of the results on daily life. Although non-verbal communication is modified by the computer presence, empathy and emotional alliance will also be able to develop during meetings and allow the establishment of an therapeutic alliance. We specify the modalities of intervention in CACR applied to depression, the digital issues in this context and the role of the therapist in this care. |
