Summary, etc. Cet article passe en revue les évolutions négatives de la psychiatrie publique en France depuis une trentaine d’années. Il essaie de montrer leur décalage par rapport aux acquis de la spécialité durant les XIXe et XXe siècles, à partir d’un rappel des positions et des écrits de plusieurs maîtres de la discipline. La psychiatrie hospitalière s’est ainsi détachée au XXIe siècle de ses racines historiques, en voulant s’aligner sur les autres spécialités médicales : fermeture massive de lits dans les services, généralisation des chambres d’isolement, rupture de la continuité des soins, transfert au médico-social d’une grande partie des missions du secteur, contraintes administratives nouvelles liées aux hospitalisations sans consentement, dépendance des praticiens et des infirmiers vis-à-vis des directions hospitalières, impact sur la pratique quotidienne d’une réglementation pesante et de multiples procédures de contrôle, informatisation à marche forcée. En perdant peu à peu tout ce qui faisait sa spécificité, la psychiatrie française s’est montrée incapable de s’adapter aux mutations socio-économiques récentes.

Summary, etc. This article reviews the negative developments in public psychiatry in France over the last thirty years. It attempts to show how they are out the step with the achievements of the specialty during the 19th and 20th centuries, based on a review of the positions and writings of several masters of the discipline. In the 21st century, hospital psychiatry has become detached from its historical roots, in an attempt to align itself with other medical specialties : massive closure of beds in the departments, generalization of isolation rooms, breakdown of continuity of care, transfer to the medico-social sector of a large part of the psychiatric sector’s missions, new administrative constraints linked to hospitalization without consent, dependence of practitioners and nurses on hospital management, impact on daily practice of cumbersome regulations and multiple control procedures, forced computerization. By gradually losing everything that made its specificity, French psychiatry has shown itself incapable of adapting to recent socio-economic changes.
