| Summary, etc. | RésuméL’intérêt pour les théories et l’exercice de la pratique analytique, l’expérience des visites à domicile, l’utilisation dans ce cas précis des écrits de S. Fraiberg, dont les comptes rendus des thérapies à domicile dans des familles « à hauts risques » physiques et psychiques pour le bébé, ont été des outils qui ont soutenu la liberté de ce suivi de trois ans d’une adolescente et de son bébé. S’y ajoutent la pratique de l’observation Esther Bick en institution ainsi que celle des thérapies groupales institutionnelles ; diverses approches qui ont été d’une aide certaine pour trouver la manière la plus adaptée d’entrer en relation avec cette famille particulièrement persécutée afin d’établir avec elle « une alliance thérapeutique ». Cette alliance, au-delà de la méfiance et des ruptures de liens, en a garanti la durée et l’évolution. Le travail qui suit s’inscrit dans les recherches faites sur la prévention des carences liées aux premiers mois de l’enfant. |

| Summary, etc. | Progressive elaboration of a therapeutic approach in the home of an adolescent motherAn interest in analytical theories and practice, the experience of home visits and the use in this particular case of S. Fraiberg’s writings including accounts of therapies at the homes of families presenting high physical and psychic risk for their babies, were among the tools which helped make the three-year accompaniment of an adolescent mother and her baby possible. Added to them were Esther Bick’s observation technique in institutions as well as institutional group therapies. These diverse approaches were definitely helpful in finding the most adapted way to enter into a relationship with this very persecuted family so as to establish a therapeutic alliance with its members. This alliance, above and beyond the distrust and the moments of disbonding, made it possible for the work to continue and evolve through time. The following article is a contribution to research done toward the prevention of physical and emotional deprivation during the first months of the child’s life. |

| Summary, etc. | ResumenEl ejercicio de la práctica analítica y el interés por sus teorías, las visitas a domicilio y la utilización de los trabajos de S. Fraiberg sobre las terapias a domicilio en familias de « alto riesgo » –físico o psíquico– para el bebé, son los instrumentos que nos han proporcionado un cierto margen de libertad durante los 3 años de seguimiento de una adolescente y de su bebé; habría que añadir la práctica de la observación en institución de Esther Bick y las terapias grupales institucionales. Todos estos puntos de vista nos han ayudado a decidir cual podía ser la forma más apropiada de entrar en contacto con una familia particularmente desconfiada y establecer con ella una « alianza terapéutica ». Esta alianza, más allá de la desconfianza y de la ruptura de los vínculos, ha garantizado la duración y la evolución de la terapia. El trabajo presentado pertenece al campo de las investigaciones sobre la prevención de las carencias relacionadas con los primeros meses del niño. |
