| Summary, etc. | Lorsqu’un adolescent est confronté à une maladie somatique grave, deux traumatismes coïncident et s’intensifient. La fantasmatique œdipienne, et d’autant plus sa composante meurtrière, va tout particulièrement s’exacerber. Instaurer un traitement psychothérapeutique dans ces circonstances est peu aisé, l’adolescent étant généralement peu demandeur et pris par ses motions agressives qu’il a tendance à accumuler tacitement et à retourner contre lui. En nous appuyant sur quelques extraits de suivis, nous allons commenter les problématiques et processus psychiques mobilisés. Nous soulignerons à quel point la verbalisation des fantasmes meurtriers est un moteur de la psychothérapie, tant pour favoriser le déploiement du processus thérapeutique que pour soulager le patient et contribuer à un nouvel équilibre psychosomatique. |

| Summary, etc. | Somatic disease and pubertal, a coincidence of traumatisms – An adolescent with a severe somatic disease is face to two traumatisms: the “pubertal process” (Gutton, 1991) and the somatic trouble. In this context, oedipal fantasies, and above all fantasies of murder tend to increase. To establish a psychotherapy in this situation is not easy: often, the adolescent does not ask a psychical treatment and is submerged by aggressive drives that he accumulates in silence and turns against himself. With some clinical cases, we will observe the psychic process involved. We will see that to express fantasies of murder helps to improve therapeutic effects, to relieve the patient and build a new psychosomatic balance. |
