Quelles sont les caractéristiques et les spécificités du type de prise en charge qu’il est amené à faire? Comment sa pratique quotidienne s’articule-t-elle avec le modèle psychanalytique? A travers le récit de la rencontre et du cheminement avec une enfant souffrant d’une grave maladie oncologique, l’auteur tente de montrer la difficulté de créer des liens et de donner du sens dans les situations extrêmes de la maladie à risque vital. Il apparaît nécessaire, dans ce type de situations, d’inventer des aménagements techniques du cadre psychothérapeutique standard. Ces aménagements, et en particulier une souplesse adaptative dans le maniement de ce même cadre, visent à permettre l’expression des expériences émotionnelles brutes que la maladie fait vivre à l’enfant et à favoriser le cheminement d’un traumatisme impensable vers une mise en jeux et en mots du vécu traumatique de la maladie. |

| Summary, etc. | The author, a Child psychiatrist working in the Geneva Childrens’ Hospital, describes some characteristics and specificities of her work with physically ill children and illustrates them with a clinical case. Inspired by psychoanalytic thought, the author describes the ways through which she related to a child suffering from a malignant cancer. In this kind of situation, when the suffering is too great to be mentally contained and when death seems on the way, creating links and giving meanings are almost impossible. The paper emphasizes the need, in such cases, to adapt the standard psychotherapeutic setting. Theses modifications seem necessary to sustain the dreaming ability of the therapeutic couple and to transform, as far as possible, the uncontained emotional experiences of the primarily unthinkable trauma into a potentially meaningful crisis. |
