| Summary, etc. | RésuméL’anorexie mentale est typique par son éclosion à l’adolescence et sa clinique stéréotypée. L’approche psychanalytique cherche, derrière le caractère identique et répété des symptômes, la dynamique qui détermine la relation d’objet et les investissements émotionnels. Dans la littérature psychanalytique, l’anorexie s’étend sur tout le champ nosologique et se classe tantôt dans les névroses, tantôt dans la dépression, les troubles de la personnalité ou les psychoses. Dans ce texte à propos d’un cas clinique, l’anorexie est considérée comme un trouble psychosomatique. Sa compréhension est basée sur les travaux de l’école psychosomatique de Paris et celles de P. Sifneos. Dans ce cas où le fonctionnement psychique de la patiente est dominé par l’alexithymie et la pensée opératoire, la psychothérapie psychanalytique classique s’est avérée insuffisante. La thérapeute, prenant en compte aussi la dynamique particulière de l’adolescence, a dû changer la technique classique et s’orienter vers de nouveaux buts. |

| Summary, etc. | The onset of anorexia nervosa occurs in adolescence and is characterized by a typical identifiable clinical picture. The psychoanalytic approach pursues behind the identical as well as repetitive elements of symptoms, the particular dynamics that determine the relational mode and emotional investments. In psychoanalytic literature anorexia covers an entire nosological field and it is either referred to as a neurosis, depression, personality disorder or psychosis. In this paper a clinical case is presented, and it is shown that anorexia has been confronted as a psychosomatic disorganization. The theoretical approach is based on P. Sifneos ideas, as well as those of the Psychosomatic School of Paris. In this particular case where the psychic functions of the patient are dominated by alexithymia and functional thought the traditional psychoanalytic therapeutic approach has seemed insufficient. The therapist, considering also the particularities of adolescence dynamics, ought to change the classical therapeutic approach and turn to different goals. |
