| Summary, etc. | RésuméCet article a pour objectif de décrire le suivi psychiatrique de patientes anorexiques assez gravement dénutries pour nécessiter une hospitalisation dans un service de médecine interne. La mise en perspective de l’entretien préliminaire permet d’abord, au travers d’un bref survol historique, de recenser les difficultés (notamment l’ambivalence et l’attitude de défi) auxquelles risque de s’achopper le processus thérapeutique qui est en train de se nouer. Ensuite, nous nous emploierons à dégager quelques points forts de la prise en charge, en insistant sur l’importance d’un travail multidisciplinaire (et même multidimensionnel) qui devrait, selon nous, inclure en tout cas une approche systémique soutenue, malgré les tensions qui peuvent en résulter dans la confrontation avec une pratique courante. Un tel abord impose de se situer sur différents plans corrélatifs, et donc de tolérer la frustration qui découle nécessairement de la prise en compte d’une certaine complexité. Nous avons décidé de conclure par une vignette clinique, afin d’illustrer les problèmes spécifiques à ce type approche. |

| Summary, etc. | This paper’s aim is to describe the psychiatric care of anorexic patients whose health is sufficiently impaired to necessitate hospitalisation in a general health medical service. The preliminary interviews with its brief historical survey puts into perspective the difficulties (especially of ambivalence and defiance). But the recently engaged therapeutic process may stumble. Then we will outline some story points in the treatment, multidisciplinary and even multidimentional work which should from our viewpoint include a thorough systemic approach in spite of tension arising with more current practices. This calls for differentiating correlatif plans and allowing the necessary frustrations of a complex nature to be better tolerated. We have decided to conclude with a clinical vignette so as to illustrate the specific problem if this type of approach. |
