| Summary, etc. | According to D.Winnicott, the need for illusion is a primary human need, followed by a progressive disillusion; it highlights the dangers of traumatic disillusion. My practice as a psychologist-psychoanalyst in maternity and neonatology allowed me to look into the concrete conditions for establishing this inaugural process of psychic functioning. In this text I propose the hypothesis that perinatal depression is the clinical expression of a failure of this process, in support of the clinic. Due to the particularities of the functioning of memory at this early age, grasping the “après-coup” of these traces in the analysis of patients at other ages of life requires joint consideration of language and modes of communication before words, as a clinical vignette of adolescence will illustrate. |

| Summary, etc. | Selon D.Winnicott, le besoin d’illusion est un besoin primaire de l’être humain, suivi d’une progressive désillusion ; il souligne les dangers d’une désillusion traumatique. Ma pratique de psychologue-psychanalyste en maternité et néonatologie m’a permis de me pencher sur les conditions concrètes d’établissement de ce processus inaugural du fonctionnement psychique. Dans ce texte, je propose l’hypothèse que la dépression périnatale est la traduction clinique d’un échec de ce processus, à l’appui de la clinique. En raison des particularités du fonctionnement de la mémoire à cet âge précoce, saisir l’après-coup de ces traces dans l’analyse de patients à d’autres âges de la vie passe par une prise en compte conjointe du langage et des modes de communication avant les mots, comme l’illustrera une vignette clinique de l’adolescence. |

Mijn praktijk als psycholoog-psychoanalyticus in kraamafdelingen en neonatologie heeft mij in staat gesteld de concrete omstandigheden te onderzoeken waarin dit inaugurele proces van psychisch functioneren tot stand komt. In deze tekst stel ik de hypothese voor dat perinatale depressie de klinische vertaling is van een mislukking in dit proces, gebaseerd op klinische gegevens. Vanwege de bijzonderheden van de manier waarop het geheugen op deze jonge leeftijd functioneert, is het voor het begrijpen van de nawerkingen van deze sporen in de analyse van patiënten in andere levensfasen nodig om rekening te houden met taal en manieren van communiceren voorafgaand aan woorden, zoals een klinisch vignet uit de adolescentie zal illustreren. |
