L’une des pistes suggérée – autonomiser leur gestion en établissant un lien direct entre activité réelle et financement – doit être interrogée. L’hôpital public, où l’activité est désormais monétarisée grâce à l’entrée en vigueur de la tarification à l’activité, montre en effet qu’il y a une contradiction entre encouragements à la productivité au niveau local et contrôle des dépenses au niveau national, qu’une concurrence émerge – entre établissements et au sein de ces derniers – plutôt que des coopérations et des fusions, et que le principal gisement de productivité – la masse salariale – est inactivé parce que la quasi totalité du personnel est composée de fonctionnaires.

Lessons Learned from the Implementation of the Price per Activity System at a Hospital — The productivity of public services is a recurring topic of debate. One of the solutions suggested – empowering their management capacities by creating a direct link between real activity and financing – needs to be re examined. The activities carried out in public hospitals are now financed by the entry into force of the price per activity system. This in turn has established that there is a contradiction between encouraging productivity at the local level and managing expenditure at the national level. Furthermore, this system incites competition – between establishments and within them – rather than cooperation and amalgamation. Lastly, it has evidenced that the main potential for gains in regards to productivity – payroll – has been exhausted because almost all employees are civil servants.
