| Summary, etc. | Néma se présente avec plusieurs cancers, dont l’équipe de psychodrame qui la suit depuis un an et demi à l’IPSO n’est pas en mesure d’apprécier la réalité, encore moins la gravité. Partant de là, avec la mise en perspective de l’histoire traumatique de Néma, deux hypothèses diagnostiques sont abordées : celle d’une psychose froide, telle que décrite par Évelyne Kestenberg, où tout investissement de l’objet analyste ne peut se faire qu’à l’abri de sa fétichisation, via le gel des ressentis (« il ne se passe rien ») ; celle d’une forme de culture du négatif à l’abri d’un clivage tel que reproduit au cours des séances dans l’avant et l’après du jeu. La dimension psychosomatique et son éventuelle « balance » d’avec le fonctionnement psychotique particulier de Néma reste difficile à appréhender dans ce contexte du « comme si », pour l’équipe malmenée dans son contre-transfert par le fonctionnement de la patiente. |

Given the traumatic history of Néma, two diagnostic hypotheses are reached : the first is that of a “cold” psychosis as has been described by Evelyne Kestenberg where any cathexis of the object-analyst can only take place when it is protected from fetishization through freezing the affects (“nothing’s happening”); the second involves a form of culture of the negative sheltered from splitting such as it was reproduced in the course of the sessions before and after the psychodrama. The psycho-somatic dimension and its possible balance with Néma’s particular psychotic functioning remain difficult for the therapeutic team to apprehend in this context of “as if”, the patient’s functioning producing countertransferential difficulties. |

A partir de la historia traumática de Néma, abordamos dos hipótesis diagnosticas, una psicosis fría como la describe E. Kestenberg, en la que toda catexis del « objeto analista » solo se puede hacer al abrigo de su fetichisación por el bloqueo del sentimiento, « no pasa nada », y una forma de cultivo de lo negativo al abrigo de un escisión, como reproduce en las sesiones en el antes y después del juego. La dimensión psicosomática y su eventual « balance » con el funcionamiento psicótico particular de Néma sigue siendo difícil de comprender, en el contexto del « como si », por un equipo maltratado en su contratransferencia por el funcionamiento del paciente. |

Das Psychodrama-Team, das sie seit ein Jahr und sex Monaten am IPSO beobachtet, ist nicht in der Lage, die Wirklichkeit, geschweige denn die Schwere dieser Krankheiten zu ermessen. Von hier aus werden zwei diagnostische Hypothesen in Bezug auf Némas traumatische Geschichte besprochen : erstens die kalte Psychose, wie sie Évelyne Kestenberg beschrieben hat, wo jede Besetzung des Objekts Analytiker nur im Schutze dessen Fetichisierung erfolgen kann, wobei alles Empfundene erforen ist (« es geschieht nichts ») ; zweitens eine Art Kultur des Negativen im Schutze einer Spaltung, die im Laufe der Analysestunden vor und nach dem Spiel wiederholt wird. Das Psychosomatische und dessen eventuelle « Balance » zu Némas besonderer psychotischer Vorgehensweise bleibt im Kontext des « Als ob » schwierig zu erfassen, zumal die Gegenübertragung des Teams durch die Vorgehensweise der Patientin erschwert wird. |
