Summary, etc. Parler de « système de santé » pour désigner un secteur sanitaire français historiquement balkanisé, cloisonné, dans lequel les services de l’État et de l’Assurance-maladie peinaient à réguler les stratégies inflationnistes des acteurs professionnels et politiques, a longtemps relevé de l’abus de langage. En contrepoint de la littérature internationale sur l’introduction de logiques de concurrence et de privatisation dans les systèmes de santé, et à partir du cas des Agences régionales, l’article soutient que la « désorganisation » sanitaire française est en proie non à l’atomisation marchande mais à une dynamique multiforme d’intégration faisant émerger des acteurs à la fois plus massifs mais aussi liés par des relations plus denses. Cette intégration se fait cependant sous forte contrainte institutionnelle.

Summary, etc. Bringing the Health Care State Back inTalking about a healthcare “system” for a French health sector historically fragmented, compartmentalized and in which State bureaucracies and sickness funds struggled to regulate professional and political strategies, has long been a misnomer. As a counterpoint to the international literature on the introduction of competition and privatization logics in healthcare systems, and based on the case of the Regional Health Agencies, the article argues that the “disorganization” of French healthcare sector is confronted not to market fragmentation but rather to a multifaceted integration process, bringing out actors both more massive but also linked by dense relations. This integration process is however under strong institutional constraints which limit its scope and fix its content.
