| Summary, etc. | À l’aide d’une sociologie des passeurs, cet article propose d’analyser le transfert, puis la consolidation des normes du « soin de la précarité » vers l’hôpital public français. Une première phase d’exportation des normes du champ associatif vers l’hôpital public (1990-1998) met en scène des « passeurs de conviction », occupant une position éminente à cheval entre le monde associatif et le monde hospitalier, et aboutit à l’institutionnalisation des normes sous la forme des permanences d’accès aux soins de santé (PASS). La seconde phase de consolidation, de défense et d’adaptation des normes (2000-2015), se déroule au sein de l’institution hospitalière et met en scène des professionnels des PASS. L’observation des passeurs et de leurs initiatives permet de mieux comprendre ce que la construction d’une politique de santé publique doit aux scansions du transfert puis de l’institutionnalisation, qui impliquent des acteurs aux profils sociologiques différents, soucieux de convaincre des publics tout aussi différents d’une phase à l’autre. |

THE TRANSFER AND REINFORCEMENT OF HEALTH CARE NORMS PERTAINING TO SOCIAL VULNERABILITY IN FRENCH PUBLIC HOSPITALSDrawing on the concept of « passeurs », as actors facilitating the circulation of norms, this article analyzes both the transfer and consolidation of health care norms pertaining to social vulnerability in French public hospitals. The initial export phase of the norms from the humanitarian field (1990-1998) involved passeurs with a strong conviction, occupying an eminent position in both, the humanitarian and hospital fields. This process led to their institutionalization in form of units dedicated to the access of health care for vulnerable patient in 1998. The second phase of consolidation and translation of the norms (2000-2015) took place within the hospitals involving PASS professionals. Passeurs of the second phase consolidated the transfer according to two modes, either by preserving the norms as they were bequeathed by the first generation of passeurs, or by adapting them to the neo-managerial standards prevailing in French hospitals at the time. The article shows that passeurs differ according to their (individual or collective) strategies and their arenas (professional association, conferences). |
