Summary, etc. The resurgence of the Covid-19 epidemic in the fall of 2020 in France and in many countries around the world raises many questions. The situation of SARS-CoV-2 infection in France after the first epidemic wave in spring 2020 approximatively indicates more than 30,000 deaths, 3 to 4 millions people infected, 50% asymptomatic infections. These data encourage us to modify the initial perception of this infection, which was imagined to be benign, with massive, homogeneous and rapid distribution (“tsunamic”), and comprising a large majority of asymptomatic forms. This invites us to reassess the hypothesis of a major role of superspreaders in the spread of the infection, which would be more limited and discontinuous (“saltatory”) than expected, as for SARS-CoV-1 and MERS-CoV. The role of viral load in the transmission and clinical expression of infection also needs to be assessed. To fight against the spread of the epidemic, generalized confinement a posteriori appears to have a disproportionate cost compared to its effectiveness, whereas the application of barrier gestures (breathing mask, hand hygiene, social distancing) should be promoted without any restriction, along with the diagnosis and temporary isolation of infected persons. While the Covid-19 epidemic is a medical challenge for human societies, it is also a moral challenge that they may not ignore.

Summary, etc. La recrudescence de l’épidémie de Covid-19 à l’automne 2020 en France et dans de nombreux pays du monde suscite de nombreuses questions. La situation de l’infection à SARS-CoV-2 en France après la première vague épidémique du printemps 2020 indique, de façon très approximative, plus de 30 000 décès, 3 à 4 millions de personnes infectées, 50 % d’infections asymptomatiques. Ces données incitent à revoir la perception initiale de cette infection que l’on imaginait bénigne, à diffusion massive, homogène et rapide (« tsunamique »), et comportant une large majorité de formes asymptomatiques. Cela invite à reconsidérer l’hypothèse d’un rôle majeur de sujets superdisséminateurs dans la propagation de l’infection qui serait plus limitée et discontinue (« saltatoire »), comme pour le SARS-CoV-1 et le MERS-CoV. Le rôle de la charge virale dans la transmission et l’expression clinique de l’infection est aussi à évaluer. Pour lutter contre la propagation de l’épidémie, le confinement généralisé apparaît a posteriori d’un coût démesuré par rapport à son efficacité alors que l’application des gestes barrières (masque respiratoire, hygiène des mains, distanciation sociale) est à promouvoir sans aucune restriction, parallèlement au diagnostic et à l’isolement temporaire des personnes infectées. Si l’épidémie de Covid-19 est un défi médical pour les sociétés humaines, elle est aussi un défi moral qu’elles ne peuvent ignorer.
