Summary, etc. | À la fin des années 1970, alors que la profession psychiatrique américaine était menacée de perdre sa légitimité vis à vis du monde dans lequel elle évoluait, le DSM-III a permis de renverser la situation. La classification fournit à tous un langage commun. Ainsi, le DSM organise les relations de la profession psychiatrique américaine avec l’ensemble de ses partenaires et rivaux. C’est en quoi cette classification diffère des autres. Ce résultat a été obtenu grâce à la priorité absolue accordée à la fiabilité, aux dépens de la validité. Notre critique du DSM ne porte pas sur son contenu, mais sur le renoncement à la validité. Nous pouvons nous libérer du DSM d’une part en travaillant à des classifications valides, et d’autre part en réhabilitant la conduite des récits psychiatriques des histoires de vie dans la longue durée, interdits par la description syndromique des troubles dans les DSM. |

Summary, etc. | The DSM and How to Throw off its Shackles. At the end of the 1970s, at a time when the American psychiatry profession was threatened with a loss of legitimacy with respect to the world in which it was evolving, the DSM-III provided it with the means of reversing this situation. The classification gave American psychiatrists a common language and thereby provided the means for structuring relations between the American psychiatry profession and its partners and rivals. This is what makes the classification different from all others. These results were obtained by granting absolute priority to reliability, rather than to validity. The criticism levelled at the DSM in this paper does not target its content, but rather the way in which it renounces the question of validity. We may free ourselves of the shackles of the DSM by, on one hand, working on valid classifications and, on the other, rehabilitating the value of long-term psychiatric accounts of mental health sufferers’ life stories, a practice which is prohibited by the purely syndrome-based description of disorders in the DSMs. |
