| Summary, etc. | Cerebral small vessel diseases are frequent brain disorders at risk of both ischemic and haemorrhagic strokes. During their course, brain MRI shows white matter hyperintensities, lacunes and/or microbleeds years before the occurrence of stroke. The consideration of these MRI markers for the management of treatments in the elderly has considerably evolved in recent years. The objective of this short review is to illustrate current treatments, which depend very much on the context, from the fortuitous discovery on brain MRI of markers of cerebral small vessel diseases until the secondary prevention in patients with a history of ischemic or haemorrhagic stroke. |

| Summary, etc. | Les maladies des petits vaisseaux cérébraux sont des affections fréquentes exposant au risque d’accidents vasculaires cérébraux (AVC) ischémique et hémorragique. Ces affections peuvent être détectées, à l’aide d’une imagerie par résonance magnétique (IRM) cérébrale des années avant la survenue de l’AVC, sous la forme d’hypersignaux de la substance blanche, de lacunes et/ou de microsaignements. L’utilisation de ces marqueurs pour la gestion des traitements chez des sujets âgés à haut risques ischémique et hémorragique a beaucoup évolué ces dernières années. L’objectif de cette mise au point est d’illustrer les prises en charge actuelles, depuis la découverte fortuite sur une IRM de marqueurs de maladies des petits vaisseaux cérébraux jusqu’à la prise en charge après un AVC ischémique ou hémorragique. |
