Summary, etc. | We report the observation of an acute coronary syndrome in an 84-year-old woman evolving to an anterior myocardial infarction with persistent elevation of the ST segment (STEMI), without significant stenosis of the left descending coronary artery after manual aspiration of a distal thrombus. The diagnostic workflow secondary to this clinical condition of MINOCA (Myocardial Infarction with Non Obstructive Coronary Arteries), integrated, according to the very latest definition of myocardial infarction, in the infarcts subgroup type 2 (not linked to an athero-thrombotic event) led to the very high probability of the diagnosis of coronary embolism related to a chronic and untreated atrial fibrillation. The purpose of this article is to provide a formal and updated definition for the broadly labelled term MINOCA (incorporating the definition of AMI from the newly released “Fourth Universal Definition of Myocardial Infarction”) and to provide a clinically useful framework and algorithms pertaining to the diagnostic evaluation and management of these patients. |

Summary, etc. | Nous rapportons l'observation d'un syndrome coronaire aigu chez une femme de 84 ans, évoluant vers un infarctus avec sus-décalage persistant du segment ST (STEMI), sans sténose significative du réseau coronaire après aspiration manuelle d'un thrombus distal de l'artère interventriculaire antérieure (IVA). Le processus diagnostique secondaire à ce tableau de MINOCA (Myocardial Infarction with Non Obstructive Coronary Arteries ou infarctus sans sténose significative des artères coronaires), s'intégrant, selon la toute dernière définition de l'infarctus du myocarde, dans le sous-groupe des infarctus de type 2 (non lié à un événement athérothrombotique) a conduit à la très forte probabilité du diagnostic d'embolie coronaire sur une fibrillation auriculaire chronique et non traitée. à cette occasion, nous précisons les définitions et recommandations les plus actuelles des sociétés savantes européenne et nord-américaine sur l'infarctus, le MINOCA et ses étiologies, dont sont maintenant exclus les myocardites et le syndrome de Takotsubo, s'intégrant quant à eux dans le « nouveau » concept de « myocardial injury » (lésions ou dommages myocardiques), caractérisé par une variation dynamique de la troponine cardiaque d'origine non ischémique. |
