Partant de l’adage d’Hermann Simon selon lequel il conviendrait de soigner l’institution avant de vouloir soigner les « malades », cet article se soutient d’une distinction fondamentale pensée par la psychothérapie institutionnelle, celle de l’établissement et de l’institution. Dans le sillage de ce repérage, quelques opérateurs majeurs proposés par François Tosquelles, Félix Guattari et Jean Oury sont rappelés, ceci afin de souligner leur fonction pour penser la clinique dans une structure de soins, et ainsi soutenir leur vitalité dans l’espace contemporain. Ce faisant, l’article aborde les effets qui résultent de la superposition des paysages de l’institution – lieu de soin et de mouvement – et de l’établissement – réalité matérielle irriguée par une logique administrativo-logistique. Enfin, une réflexion est esquissée quant à la libre circulation… du clinicien. C’est parce que l’institution est mobilisée par les sujets qui s’y logent, s’y invitent, y surgissent – bousculant l’installé, le confortable et l’organisé – qu’elle est en mesure de soigner… l’établissement.

Starting from the adage of Hermann Simon according to which it would be better to treat the institution before seeking to treat the “patients”, this article draws on a fundamental distinction made by institutional psychotherapy between the establishment and the institution. In the wake of this distinction, a number of major operators proposed by François Tosquelles, Félix Guattari and Jean Oury are recalled in order to underline their function for thinking through clinical work within a treatment structure, and thus to support their vitality in the contemporary space. The article thereby discusses the effects that result from superimposing the landscapes of the institution (a place of treatment and movement) and of the establishment (material reality irrigated by and administrative-logistical logic. Finally, some thoughts are outlined concerning the free circulation… of the clinician. It is because the institution is mobilised by the subjects who live there, invite themselves, or suddenly appear – upsetting what is established, comfortable and organised – that it is able to treat… the establishment.

Partiendo del refrán de Hermann Simon según el cual convendría curar la institución antes de querer curar a los “enfermos”, este artículo se apoya en una distinción fundamental pensada por la psicoterapia institucional, la del establecimiento y de la institución. En la estela de esta identificación, varios operadores mayores propuestos por François Tosquelles, Félix Guattari y Jean Oury serán recordados para subrayar su función para pensar la clínica en un centro de atención, y así sostener su vitalidad en el espacio contemporáneo. Haciendo esto, el artículo aborda los efectos que resultan de la superposición de paisajes de la institución- lugar de curación y de movimiento – y del establecimiento – realidad material irrigada por una lógica administrativa-logística. Finalmente, una reflexión se deslumbra en relación a la libre circulación…del clínico. Gracias a la movilización de la institución por el sujeto, allí se ubican, se instalan y surgen – movilizando lo instalado, lo confortable y lo organizado –, ella está preparada para curar…al establecimiento.
