Studies have noted a high prevalence of substance use in people with schizophrenia. We present the case of a forty-year-old Moroccan patient with schizophrenia who was admitted to our training center for arson behavior. The patient’s inpatient care and clinical progression revealed that the arson behaviors were not linked to schizophrenia, but rather to a comorbid substance use disorder, specifically the inhalation of smoke from the combustion of organic and non-organic materials. In light of this clinical case and the literature review, we highlight the importance of clinical assessment in identifying this comorbidity to better understand it and develop more effective preventive interventions and treatments.
