Child psychiatry for an anorexic baby and her mother: Creating intimacy

An intimate bond between Kika and her mother initially seemed impossible. Kika avoided it, wanting nothing from the Other. She presented with a polymalformative syndrome that had been the subject of numerous medical investigations since birth. Pediatricians referred her to us, for treatment for complex and persistent anorexia. Through the reconstruction of this clinical history, we will describe the various care spaces, or “scenes,” in which Kika and her mother were able to evolve: the co-therapy space, the space of the “baby group” in pediatrics, the space of reception within the UTEPP's therapeutic nursery, and the talking group space. In this context of babies who have severe problems during their early development and whose parents who are overwhelmed, we will attempt to demonstrate a method of care that involves multiple “scenes” and uses video as part of a psychodynamic approach. This detour through the image allows therapists to avoid being sucked into the psychological void themselves and keeps their own thinking abilities alive. The moving image captures the process of developmental induction, both for the emergence of the baby's psychological life and for the resumption of the parents’ involvement.
