The hospital-to-city transition and patients’ return to their usual environment after being hospitalized for addiction withdrawal is a crucial point in time: there are sometimes problems with managing drug intake at home; and there is a potential for non-compliance and interruption of treatment or treatment misuse, leading to them resuming their consumption of psychoactive substances and “relapsing,” as well as struggling to keep follow-up appointments with the attending physician. The continuing drug treatment must be supervised because of the communication difficulties between the hospital and the city doctor/pharmacy.
