The transfer from pediatrics to adult medicine is an essential step for adolescents and young adults living with a chronic disease. It involves a significant risk of disruption of the care pathway with harmful consequences for health in the short and long term. The challenge of this transition is to ensure continuity of care while enabling the adolescent to successfully reach developmental objectives, in particular autonomy. The success of this transfer therefore requires organized and structured preparation, tailored for each patient and their parents, over a transitional period that begins several years before transfer and extends to the establishment of regular follow-up care in an adult service.
