The sense of continuity during the perinatal period. Concepts and tools: A slow development

As a foundation in the construction of the human being, the means of ensuring it in the child under construction involve many factors. The entry of attachment theory into medical and psychological culture has promoted its diffusion. Initially studied within the mother–baby dyad, the analysis framework has expanded to situate the father and the professional environment in its proximity to parents and the child at early stages of life, marked by the medicalization of previous decades. As a result, the obstetric–pediatric world has become more complex. New health care technologies have confronted teams with emotional loads that are unavoidable. It is by bringing together the disciplines concerned with the child’s future, both somatically and psycho-emotionally, that it has been possible to identify gaps in the professional world. These differences stem from the materiality of the organizations, the far-reaching conditions of exercise, the previously exclusive mono-disciplinary cultures, as well as the languages and the distinct objectives. The impact of medical and psychosocial attitudes and practices in these sensitive moments has become the object of research. Each field develops its strategies, assisted according to the site by a care policy such as the creation of the Perinatal Networks in France. A close collaboration between three teams, French, Swiss, and Belgian, has facilitated the careful analysis over two decades of the processes of psycho-emotional change in the future parents, and their impact on the child in the making, supported by new methods of multi-professional support throughout every stage. Even though health policies differ, a common mindset emerged, allowing for the inventiveness and pace of each site to develop. A collaborative publication and teaching project was conducted. The “APRC” scheme (personalized support in a coordinated network) developed over the years thanks to the proposals of committed practitioners, and to the devices that organize communication within the network of professionals. Elements of discontinuity are present, necessary, and testify to the diversification of roles over the course of each stage. However, they fit into an ever-changing environmental context, forming a protective and flexible canvas around people who can rely on it. Set up on the Montpellier site, the APRC recently benefited from a comparative study that confirms its effectiveness. It is a step in the early prevention effort, which can be permanently improved.
