Context and Objectives: In the context of bipolar disorder, non-adherence to treatments, whether medicinal or otherwise, represents a major health issue. Therefore, we undertook to evaluate therapeutic adherence profiles and levels of insight among bipolar patients, as well as to study the relationship between these two aspects. Materials and Methods: This was a cross-sectional study for descriptive and analytical purposes, conducted at the Mohammed VI University Hospital for Mental Health and Psychiatric Illnesses in Oujda over a period of 4 months. The study focused on patients with bipolar disorder type I in euthymic state. The Medication Adherence Report Scale (MARS-5) and Birchwood Insight Scale were used as measurement tools. Statistical analysis was performed using SPSS version 21.0. Our study was approved by the Biomedical Research Ethics Committee of the Faculty of Medicine and Pharmacy of Oujda. Results: Among the 35 patients included in the study, 65.7% were considered adherent to their treatment, while 34.3% were not. Furthermore, 80% showed good insight, with the dimension most affected by poor insight being awareness of the illness. Patients were more likely to be adherent if they had good insight, lacked addictive comorbidities, developed the illness at a younger age, had previous hospitalizations, and did not exhibit residual symptoms. However, despite these observations, no significant association between these factors was identified in our study. Conclusion: Managing bipolar patients involves improving their adherence, which depends, among other factors, on enhancing their insight.
