Parmi les formes de restructuration, la fusion s’est imposée aux établissements depuis le début des années 1990. Sans parvenir à démontrer son efficacité en matière de recomposition de l’offre de soins, la fusion constitue un processus de changement institutionnel. Après une période de fortes incitations, il semble que les opérations de coopération soient désormais privilégiées aux opérations de fusion. Notre article interroge, à travers l’analyse longitudinale de cinq opérations de fusion, les mécanismes de changement institutionnel à l’œuvre. Nous montrerons les apports de l’approche néo-institutionnaliste dans l’analyse des processus de changement imposés d’une part. D’autre part, nous discuterons en quoi ces approches dessinent une vision de la performance en santé au confluent entre rationalité et conformité.1

Analysis of hospital mergers over the long term: institutional changeAmong the various forms of restructuring, the merger has dominated in hospitals since the early 1990s. Although it has not proved its effectiveness in the area of reorganised provision of care, the merger is a process of institutional change. After a period with strong incentives, it seems that cooperating is now preferred to merging. By means of a longitudinal analysis of five merger operations, our article addresses the mechanisms of institutional change at work. We will demonstrate, on the one hand, the contribution of the neo-institutionalist approach in the analysis of the imposed change processes. On the other hand, we will discuss how these approaches outline a vision of performance in health care which is at the confluence of rationality and conformity.
