L’analyse de l’évolution d’un patient tétraplégique d’une soixantaine d’années au cours d’une psychothérapie permet de montrer le rôle nodal de modalités psychiques régressives, formelles et fantasmatiques. Tout en tentant de protéger le sujet d’une réalité trop insupportable, elles témoignent également qu’un travail psychique de réorganisation interne est à l’œuvre et qu’il vise, malgré les aléas qui l’animent, l’intégration du bouleversement de la grande dépendance dans l’histoire du sujet.

Analysing the evolution of a tetraplegic patient of about sixty years old during a psychotherapy allows to show the nodal role of regressive, formal and phantasmatic psychic modalities. While trying to protect the subject from an unbearable reality, they also testify that a psychic work of internal reorganization is at work and that it aims, in spite of the hazards which animate it, at the integration of the upheaval of the great dependence in the subject’s history.
