Le présent article traite de la spécificitédes groupes analytiques au sein d’uneinstitution psychiatrique. En rappelantl’utilisation du groupe par le courant dela psychothérapie institutionnelle, nousabordons en quoi le petit groupe devientespace thérapeutique pour des patientspsychotiques au long cours, pris en chargeen foyer de postcure. Une première partieaborde la mise en place d’un groupe ausein d’une institution et la double écouteanalytique qu’elle implique : celle sur lesliens d’interdépendance entre cadre institutionnel et dispositif groupal et celle surles processus groupaux au sein du groupelui-même. Une deuxième partie abordeles processus psychiques élaborés à l’intérieur du groupe à travers l’évolution engroupe thérapeutique sur cinq ans d’unepatiente psychotique au long cours.

The function of the psychoanalytic group in psychiatry The present article deals with the specificity of psychoanalytical therapeuticgroups in a psychiatric institution. Inrecalling how institutional psychotherapyuses the group, we take up the questionof the way in which the small groupbecomes a therapeutic space for chronicpsychotic patients treated inside a convalescent residence. We discuss how a therapeutic group forms inside a psychiatricinstitution and the double implications ofthis for analytical thinking : first, on theinterdependency links between institutional and group settings, and secondly,on the group processes that take placeinside the developing group. Finally,the evolution of a psychotic patient in agroup over a five-year period illustratesthe therapeutic action of groups.
