Poncet's rheumatism is an oligo- or polyarthritis in reaction to tuberculosis disease, mainly affecting the large and medium-sized joints. The diagnosis is based on the identification of a tuberculous focus in the patient and on the elimination of differential diagnoses, notably tuberculous arthritis, which is accompanied by the presence of Koch's bacillus in the joint fluid, Poncet's rheumatism being aseptic. We report the case of a 58-year-old female patient who presented with an altered general condition, enlarged and painful ankles and a lesion on the right leg. Imaging and bronchoalveolar lavage showed the presence of pulmonary tuberculosis. Four courses of anti-tuberculosis therapy led to the disappearance of the arthralgias, confirming the diagnosis of Poncet's rheumatism.

Le rhumatisme de Poncet est une oligo- ou polyarthrite réactionnelle à une tuberculose maladie touchant principalement les grosses et moyennes articulations. Son diagnostic repose sur l’identification d’un foyer tuberculeux chez le patient et sur l’élimination des diagnostics différentiels, notamment une arthrite tuberculeuse, qui s’accompagne d’une présence du bacille de Koch dans le liquide articulaire, le rhumatisme de Poncet étant aseptique. Nous rapportons le cas d’une patiente de 58 ans se présentant avec une altération de l’état général, des chevilles de volume augmenté et douloureuses et une lésion de la jambe droite. L’imagerie et le lavage bronchoalvéolaire montraient la présence d’une tuberculose pulmonaire. La quadrithérapie antituberculeuse permettait une disparition des arthralgies, confirmant le diagnostic de rhumatisme de Poncet.
