During adolescence, the instinctual drive, which can be experienced as a true “internal foreign body,” can threaten the narcissism under construction, to the point of sometimes reviving unspeakable agonistic experiences. “I’m scared that I can’t stop myself”: this is Agathe’s fear. Her symptoms (anorexia, scarification) lodge themselves within the body, which is both the site of the conflict and the tool with which to attempt to resolve it. What function can the destructive drive sometimes hold, in particular, when it is turned against the self? What is behind this relentless determination of anorexic subjects to create emptiness? These are the questions the author tries to answer through the case study of a young woman receiving follow-up care in an outpatient clinic.
