The author proposes, from her position as an adult psychiatrist, re-examining her own theoretical and clinical journey with Winnicott over the past twenty-five years. First, she comments on some of her recent works on early childhood which talk a lot about adults, doctors, nurses, and patients. Then she expounds some of the main themes of her personal understanding of the institutional work in a day hospital for adult psychotic patients, understanding greatly influenced by Winnicott’s ideas. Two vignettes illustrate this kind of institutional work.
