Tobacco addiction: Community–hospital care coordination in medical practice

In 2017, an innovative structure to support smoking cessation was established and developed at the Bouscat Suburban Hospital in Bordeaux. A coordinated care plan between addictologists and attending physicians was set up to treat patients in specialist consultations with the support of a summarized reference guide and a shared patient file. This shared approach to care provides primary care practitioners with practical experience of smoking cessation assistance in their practice. Promising results, delivered over three consecutive years, will be presented below in part two.
