Tobacco control: Historical foundations and effectiveness of prevention measures in the light of the clinical context

The dangerousness of tobacco has not always been obvious. The discovery of this fact is relatively recent, in relation to the history of the product in France. For fifty years, French law has increased the number of preventive measures against smoking, which have been fruitful: the prevalence of smoking has significantly decreased, even if it remains high. While such measures are necessary, they do not seem sufficient to help smokers quit smoking. Studying the role of tobacco in the subject’s psychic economy is necessary. We present here the analysis of two smokers’ discourses. It appears that recognizing the dangerousness of tobacco does not prevent the positive feelings associated with smoking, or the denial of its harmful effects on the body. In addition, the use of tobacco as a transitory object makes it necessary for the psychic balance of some smokers, who cannot do without it at the risk of collapsing. For these cases, psychodynamic treatment is a possible solution to overcome addiction.
