Personalizing services. . . in a timely fashion? A case of contradictory injunctions suffered by orderlies in long-term care facilities in Quebec

The objective of our article is to present a case of contradictory injunctions suffered by orderlies in geriatric organizations in Quebec. This is the obligation to provide personalized services to residents in residential long-term care centers (CHSLD by the French acroynm), while respecting the schedules prescribed by the ratio of residents to orderlies. Quality requirements are imposed by governmental and quasi-governmental authorities (such as Accreditation Canada and the Ministry of Health and Social Services) that are often removed from the organizational reality. These contradictory injunctions produce forms of ethical suffering experienced by the orderlies, who must constantly reduce their own standards in terms of quality, and who are increasingly leaving the occupation. It is therefore necessary to identify strategies to give orderlies more of a voice in the development of these prescriptions.
