This article examines the displacements induced when psychoanalytic theory takes into account the triple registration (geographical, historical and social) of subjects, or the fact that they live in a certain context. Moreover, when this registration occurs in a destructuring, or even decivilizing situation, it may be supposed that psychic processes and defence mechanisms are brought into play. The psychic economy then becomes permeable to certain configurations and certain social experiences. How are these modifications thematized and what modifications of the analytic frame have been envisaged to accommodate the suffering of these subjects? What settings have been invented to deal with these extreme situations clinically?
