A functional neurological symptom disorder in the context of the COVID-19 pandemic

Background: The COVID-19 pandemic has been accompanied by mental health consequences sometimes leading to mental disorders. While depression and anxiety seem to prevail, other clinical entities with a more complex diagnosis can be described, such as functional neurological symptom disorder (FNSD). After describing the characteristics of the different clinical pictures of FNSD, we will discuss the case of a patient who manifested a polymorphic clinical picture in the context of the COVID-19 pandemic. Case presentation: The patient was admitted to the neurology department because of neurological symptoms and cognitive complaints following COVID-19-like symptomatic manifestations without the diagnosis being confirmed by infectious markers. The patient underwent neurological, neuropsychological, psychopathological, and neuroimaging (MRI and PET) examinations. Examinations showed normal results despite physical, motor, somatosensory, sleep, and cognitive complaints. The neuropsychological assessment revealed psychomotor retardation, cognitive fatigue, and deficits in cognitive planning. Conclusion: These functional symptomatologies underline the importance of the COVID-19 pandemic’s mental health repercussions and the importance of specialized follow-up (diagnostic and therapeutic) even in individuals who have not contracted the disease.
