This article offers reflections, with reference to a group psychic treatment setting, on the problem of subjects who do not frequent treatment centres and who are found malfunctioning within society at large. These subjects, generally classified as subjects who do not seek treatment, present a specific psychic organization that requires a rigorous work of identification in order to understand their way of functioning and thinking prior to granting them access to treatment. With the help of a clinical situation retracing two years of multifocal provision of care, the author theorizes an institutional treatment setting supported by a practice of accompaniment in the community which integrates a phoric function linked to a supplementary group psychical apparatus.
