The vicissitudes of sensori-motricity and institutional symbolization in care settings for psychotic and autistic children

In a context of institutional psychotherapy for psychotic and autistic children, the transference constellation manifests itself essentially in the form of sensorimotor associativity and is organized into a chain of transferences through its deployment within different geographical zones of the institution. The mimico-gesturo-postural manifestations of the children, often interpreted as signs of major destructivity, produce a transference gap in the chain of transferences. During threshold moments of regulation, the clinician’s countertransference experience will enable him to transform these diverse facts into clinical facts, and to accompany a series of interlocking reflexive and reflecting loops in the organization of the group associative chain. The analysis of these transference/countertransference processes in the context of institutional meetings permits these children’s ego islets which are waiting to be gathered together, to link up with one another, supported by the multi-focal deployment of the countertransference chain of the care-givers. The transference of the transference thus proves to be the nodal point of institutional care based on sensori-motor productions.
