The enforcement of the January 1994 reform represents a genuine break with the past. The reform has given rise to a system of agreements between hospitals and prisons that has greatly improved medical care in prisons. But such medical care, particularly for patients with chronic conditions, is of no value unless it is continuous. The prison population exhibits a combination of social and health risk factors that make it much more vulnerable to health problems than the population in general.
