RésuméLes matricides sont rares, ils sont surtout, mais pas seulement, commis par des malades mentaux. Les matricides pathologiques sont le lot des pathologies psychiatriques lourdes, en particulier, la schizophrénie. Ces crimes ont des répercussions sociofamiliales graves, ils contribuent en grande partie à la rupture des liens familiaux et à la stigmatisation des malades mentaux. Nous présentons un cas de matricide commis par un homme de 46 ans, présentant une psychose hallucinatoire chronique. Le patient a procédé à l’étranglement de sa mère dans un cadre délirant et hallucinatoire. Le diagnostic de psychose hallucinatoire chronique est rarement rapporté dans le cadre des matricides. Ce meurtre interroge indéniablement la dangerosité des malades mentaux, dont l’acmé se résout parfois dans le passage à l’acte d’homicide intrafamilial et soulève le problème de pronostic et de prévention de la violence des malades psychotiques.
Matricide and chronic hallucinatory psychosis “case report”Matricides are rarely reported in the literature; they are mainly committed by mental patients. The pathological matricides are a set of heavy psychiatric pathologies including the schizophrenia. These crimes have severe socio-family impact; they contribute largely to the collapse of the family and to the stigmatization of the mental patients. We report a case of matricide committed by a 46-year-old man, married and a father of two children. The patient has had a well-established socio-familial and professional life. At the age of 45, the patient presented a progressive installation of psychotic symptomatology; this consisted of auditory hallucinations of persecuting nature with affronts and insults. This episode lasted six months and evolved well under classical antipsychotic treatment. On one year later, the patient demonstrated the same episode but severer, and associating a more obvious mental automatism, which directed him to kill his mother, since he considered that she is a Satan. The examination of the patient immediately after this crime found a very rich hallucinatory syndrome with a mental automatism associating lived persecution and fear. A good improvement was noticed after three months using haloperidol and olanzapine. The diagnosis of chronic hallucinatory psychosis was established, considering the delayed start, the existence of rich hallucinatory persecuting elements with an absence of any dissociation; however, this affection is rarely reported within the framework of the matricides. This murder questions obviously about the dangerousness of the mental patients. The acme is sometimes translated by acting such interfamily homicide; this raises the issue of prognosis and prevention of the violence of psychotics.
Matricidio y psicosis alucinatoria crónicaSon pocos los matricidios, y sobre todo, pero no únicamente, cometidos por enfermos mentales. Los matricidios patológicos remiten a patologías psiquiátricas severas en particular a la esquizofrenia. Estos crímenes tienen repercusiones socio-familiares graves, contribuyen en gran parte a la ruptura de los vínculos familiares y a la estigmatización de los enfermos mentales. Presentamos un caso de matricidio cometido por un hombre de 46 años con psicosis alucinatoria crónica. El paciente llevó a cabo el estrangulamiento de su madre en un cuadro delirante y alucinatorio. El diagnóstico de psicosis alucinatoria crónica rara vez se da a conocer en un cuadro de matricidios. Este homicidio interroga sin lugar a dudas acerca de la peligrosidad de los enfermos mentales cuyo acmé se resuelve a veces en el paso al acto de homicidio intrafamiliar, y plantea el problema del diagnóstico y prevención de los enfermos psicóticos.
