Nous montrons dans ce travail comment les différents dispositifs de pilotage de la performance hospitalière induits par la T2A et la Nouvelle Gouvernance contribuent à la marchandisation du soin, qui pousse les établissements à développer l’activité à moyens constants, et nous analysons les risques qu’une telle situation fait encourir au service public, en lien notamment avec le déni de la dimension humaine. Nous apportons des éléments nourrissant la réflexion sur les pistes de solution possibles : d’abord en nous penchant sur l’exemple des organisations de haute fiabilité, qui accordent une priorité à la sécurité, et ont développé une expertise dont nous pouvons apprendre ; puis en apportant des éléments plus théoriques ouvrant la voie à un renversement paradigmatique pour considérer le service public de santé comme un bien commun, tel que défini et étudié par Elinor Ostrom.

We show through this piece of work the way by which performance management devices introduced by the prospective payment system and the so-called Nouvelle Gouvernance reforms contribute to healthcare commodification that urges hospitals to develop activity with no increase of budget and we analyse the risks that such a situation induces for the healthcare public service as a whole, with a special attention granted to denying human dimensions. We propose a series of elements to nurture reflexion on possible tracks for solution : first we borrow from high reliability organizations which grant a high priority to safety and have developed an expertise from which we can learn ; then we bring more theoretical inputs opening the way towards a paradigmatic reverse that leads to consider the healthcare public service system as a commons as defined and studied by Elinor Ostrom.
