L’article analyse la conception et les transformations depuis les années 1960 d’une ingénierie financière qui est orientée vers la maîtrise des dépenses hospitalières. Il s’intéresse aux relations d’alliance qui portent ces instruments entre des médecins hospitaliers, des directeurs d’hôpital et des cadres de l’administration sanitaire. Il montre ainsi que ces instruments sont constitués à la charnière des écologies étatique et professionnelle au sein desquelles ils sont investis de différentes significations et appuient des stratégies de revendication ou de protection territoriales. Ces alliances sont appréhendées en distinguant trois séquences historiques qui analysent leur évolution et les changements d’instrument dont elles sont porteuses, d’un prix de journée sophistiqué jusqu’à l’adoption d’une tarification à l’activité.

The article analyzes the conception and the transformations of the financing tools that aim to control hospital expenditure since the 1960s. It deals with the alliances supporting these tools between physicians, hospital directors, and administrators at the French Ministry of Health. It considers these tools as a hinge between the state ecology and the professional one. These tools are given different meanings in each ecology, where they contribute to offensive and defensive jurisdictional tactics. The article distinguishes three historical sequences that explain the evolution of these alliances and the related changes of financing tools, from a sophisticated bed-day price to a prospective payment system.
