On articule la médecine génétique, la pédopsychiatrie et la psychopathologie psychanalytique pour proposer une caractérisation psychanalytique de l’impact psychique d’une pathologie développementale d’origine génétique à fort versant neurologique, la microdélétion 22q11.2. Après une caractérisation de la microdélétion, et du style relationnel des enfants qui en sont affectés, nous soulignons d’abord l’effraction du narcissisme familial que représente cette atteinte, et les voies par lesquelles la contenir en travaillant l’annonce. Nous proposons ensuite d’intégrer à la prise en charge polyfactorielle un suivi psychothérapique de la famille, et de l’enfant, centrés sur l’étayage du mouvement interactif vers autrui que l’enfant manifeste spontanément. Nous indiquons quelques pistes suivant lesquelles déployer cet étayage.

This paper brings together the perspectives of genetic medicine, child psychiatry and psychoanalytical psychopathology to characterize, from a psychoanalytic standpoint, the psychic impact of a genetic neurodevelopmental pathology (the 22q11.2 deletion syndrome). We start by recalling its clinical picture, and in particular the relational style of children that it affects. Then, we shed light on the impact of this syndrome of family narcissism, and how to contain it by modulating how the diagnosis is communicated. We then look at how best to integrate psychotherapeutic care for the family and child into a multi-factorial care programme; psychotherapy should aim at providing a transferential basis to the child’s spontaneous movement towards others. We offer a number of ideas to develop this support.
