Fever of unknown origin (FUO) remains a clinical challenge despite considerable advances in medical technology. While the proportion of undiagnosed cases has decreased, the complexity of possible etiologies has increased, making a multidisciplinary, structured, and innovative approach essential. This article reviews recent advances in the diagnosis of FUO, drawing on tools from molecular biology, high-precision imaging, artificial intelligence, and genetics. By updating current exploration strategies and examining future perspectives, we highlight the importance of an integrated approach to medicine that is both technologically driven and rooted in clinical practice.
