<br/><br/> nous avons constaté, au niveau du pôle de psychiatrie de Cayenne, tant dans le service de pédopsychiatrie que dans celui de psychiatrie adulte, que beaucoup de diagnostics trop tardifs - car les patients étaient monopolisés par les soins traditionnels - ont entraîné une perte de chance évidente et un pronostic défavorable pouvant aller jusqu’au décès, notamment dans les pathologies bipolaires ou psychotiques. Le Dr Razafindrazara montre. avec l’éclairage de sou expérience préalable en santé publique dans la prise en charge du VIH à Madagascar, comment une alliance est obligatoire avec les responsables familiaux et des structures traditionnelles afin de permettre aux patients touchés par les maladies d’avoir accès également à la médecine moderne. Le Dr Simchowitz intervient à la lumière de son expérience avec les tradipraticiens de la Réunion où les patients psychiatriques étaient pris en charge de façon adéquate par la médecine traditionnelle pour les pathologies névrotiques et réactionnelles et dirigés pour les pathologies psychotiques et bipolaires vers la médecine moderne. Un véritable partenariat entre praticiens traditionnels et psychiatres avait été établi. Il faut donc en Guyane, établir des ponts avec une circulation dans les deux sens des patients entre tradition et modernité.  in the psychiatric center of Cayenne, we noticed that too many diagnoses were lately given in the child psychiatry department and in the adult psychiatry service, beause many patients were monopolized by local traditional trearment. Patients have obviously a loss of chance to have a better treatment &amp; prognosis, wich might take them to death, especially in bipolar or psychotic pathologies. Dr. Razafindrazara shows, with the insight of his prior public heath expérience in the management of HIV treatments in Madagascar, how an alliance with Family leaders and traditional structures is able to help patients affected by HIV to have access to modern medicine. Dr. Simchowitz has an experience of traditional healers of Réunion Island where psychiatrie patients were adequately treared by traditional medicine for neurotic and reactivé diseases and directed for psychotic and bipolar pathologies towards modern medicine. A true partnership between traditional practitioners and psychiatrists had been established. In Guyana, bridges must therefore be established with a circulation of patients in both directions between tradition and modernity
