N1 - 80 N2 - Improved survival following a cancer diagnosis makes it crucial that a sustainable return to work is supported, particularly for employees in the private sector, who are at higher risk of occupational disengagement. To address this need, the Institut de Cancérologie de l’Ouest has developed an innovative hospital-based coordination programme offering individualised support, initiated within the care pathway and integrated with primary care, occupational health services and the workplace. Grounded in theory and evidence, the programme combines clinical assessment, interprofessional coordination, and standardised tools for both patients and professional stakeholders. Embedded within a structured regional network in the Pays de la Loire, the initiative aims to secure return-to-work pathways and prevent post-cancer occupational exclusion. An ongoing pilot study involving women treated for non-metastatic breast cancer will evaluate the programme’s effectiveness and implementation conditions.
