N2  - L’amélioration de la survie après le diagnostic d’un cancer rend crucial le soutien au retour durable au travail, en particulier pour les salariés du secteur privé, plus exposés au risque de désinsertion professionnelle. Pour répondre à ce besoin, l’Institut de cancérologie de l’Ouest a développé une coordination hospitalière innovante proposant un accompagnement individualisé, initié au sein du parcours de soins et articulé avec la médecine de ville, la santé au travail et l’entreprise. Fondé sur la théorie et les preuves, le dispositif combine évaluation clinique, coordination interprofessionnelle et outils standardisés destinés aux patients et aux acteurs professionnels. Inscrit dans un réseau territorial structuré en Pays de la Loire, ce dispositif vise à sécuriser les trajectoires de reprise du travail et à prévenir la désinsertion professionnelle de l’après-cancer. Une étude pilote en cours, auprès de femmes traitées pour un cancer du sein non métastatique, permettra d’en préciser l’efficacité et les conditions de déploiement. Le programme fait également l’objet d’adaptations pour d’autres types de cancers et pour les aidants; Improved survival following a cancer diagnosis makes it crucial that a sustainable return to work is supported, particularly for employees in the private sector, who are at higher risk of occupational disengagement. To address this need, the Institut de Cancérologie de l’Ouest has developed an innovative hospital-based coordination programme offering individualised support, initiated within the care pathway and integrated with primary care, occupational health services and the workplace. Grounded in theory and evidence, the programme combines clinical assessment, interprofessional coordination, and standardised tools for both patients and professional stakeholders. Embedded within a structured regional network in the Pays de la Loire, the initiative aims to secure return-to-work pathways and prevent post-cancer occupational exclusion. An ongoing pilot study involving women treated for non-metastatic breast cancer will evaluate the programme’s effectiveness and implementation conditions. The programme is also being adapted for other cancer types and for caregivers
